My Mission Statement reads, "all patients are accepted regardless of their condition or ability to pay". My hope is that the following sliding payment scale will provide everyone an affordable means for receiving high quality health care.

I also accept a number of health, auto and worker's compensation insurances. Payments can be made by cash or check. Sorry, no credit cards accepted at this time.
